Record of service is our document that lets parents or guardians confirm the work that you’ve done. For clients that you work with with the FSCD contract there is a requirement the record of service be submitted with an original signature. For us to bill for your services and for you to be paid for your services, the record of service needs an original signature that we will mail to FSCD. There should be one record of service for each client that you work with. It should be submitted no later than the 5th day of the month following services. (If you worked April 2018 you would submit your record of service on May 5, 2018)
What needs to be on your record of service
- the month and year that services were given
- the client’s name
- the caregiver’s name
- for each appointment you need
- the date
- type of service
- the start time
- the end time
- your signature, name, and the date you signed.
- The Parent/Guardian signature, name, and the date that they signed.
- IF YOU DO NOT HAVE AN ORIGINAL SIGNATURE YOU MAY NOT BE PAID
